A copy of your child's immunization record is required at time of registration, even if it's partially out of date. However, in order to start school, all immunizations must be complete and cleared by the district nurse before the first day of school.
The TB risk assessment signed by a U.S. physician or U.S. healthcare provider is due before the student can start school. Children who are new to Santa Clara county schools or are starting TK/Kindergarten are required to have the assessment done within 12 months of starting school.
